Change of name
You can apply to change your name if you are 18 years or older, and were born in Victoria or have lived in Victoria for at least the last 12 months.

You can apply to change your child's name if they are under 18 years of age, and were born in Victoria or have lived in Victoria for at least the last 12 months.
Deed Polls were used up to October 1986 to change a person's name. You can apply to BDM for a copy of your existing Victorian Deed Poll certificate.
