Information under the Child Wellbeing and Safety Act - request form

This page contains a request form that can be completed by Information Sharing Entities to request information under the Child Information Sharing Scheme.

The Child Wellbeing and Safety Act 2005 (the Act) and its Regulations permit some organisations to share confidential information to promote the wellbeing or safety of Victorian children. These organisations are known as Information Sharing Entities, or ISEs.

This form is for ISEs to request information from the Registry of Births, Deaths and Marriages Victoria (BDM) under the Act.
