Statutory declaration - home birth (witness)

The witness to a home birth must complete this statutory declaration so that the mother or parent(s) can register the birth.

If more than one child was born (e.g. twins), please fill out a separate declaration for each.

To register the birth, the mother or parent(s) should go to Register a birth and attach:

  • This statutory declaration, and
  • A letter from the health practitioner.
