Statutory declaration change of name - Parent/guardian of a child born in Victoria

A parent or guardian applying to change a child's name must attach a statutory declaration. The declaration includes that: 

• They are the parent or guardian of the child listed in the application
• The child’s birth was registered in Victoria
• They wish for the child to be known by the new name (legal name) as stated in the application.
